This service allows the applicant to request the attestation of a sick leave or escort leave issued by a healthcare facility licensed by Sharjah Health Authority.
Required Documents:
- sick leave certificate from the facility.
- Medical Report if the sick leave is more than 4 days.
- Discharge Summary.
Conditions & Requirements:
- The healthcare facility must be licensed by the Sharjah Health Authority.
- The physician must hold a valid full-time or part-time license issued by the Sharjah Health Authority.
- Healthcare professional be a specialist or consultant to issue sick leaves exceeding 4 days.
- The sick leave from date must be on the same visit date or one day after.
Service Process:
- Log in to the Sharjah Health Authority system.
- Submit the application and attach the required documents.
- Pay the application fees through the system.
- Internal review of the application by the Authority.
- Issuance of an attested sick leave certificate or attested escort leave certificate.
