- Vehicle license/ Card transaction tariffs for machinery and equipment.
- Basic Life Support Training Certificate.
- Advanced Life Support Training Certificate.
- Pediatric Advance Life Support Training Certificate.
- Contract of Providing Medical / Nursing Staff (if there is a need to avail a licensed medical / nursing staff from another medical facility).
- Medical Malpractice Insurance for the Medical Staff.
- Driver License Copy.
- Driver First Aid Training Certificate.
- Supporting Document Justified the Exemption of Fees.
- Supporting Documents (if any).
Conditions & Requirements:
- The vehicle must be fully and properly equipped with approved medical equipment.
- The vehicle must belong to a licensed healthcare facility.
- The vehicle must be specifically designated and physically equipped as an ambulance in accordance with approved technical standards.
- All devices and equipment inside the vehicle must comply with the Authority’s approved specifications and standards.
Service Process:
- Log in to the Sharjah Health Authority system.
- Fill out the online application form for initial approval.
- Attach the required documents.
- The application will be reviewed and audited by the Authority.
- Pay the fees, if applicable.
